In the General Assessment section of a vendor self-assessment checklist, fill up your Company UEN, Company Name, Solution Name

In the Satisfaction Survey worksheet, copy and send the link to all the SMEs who are performing the surveys.

To complete your customer satisfaction surveys:
Share the FormSG survey link with your customers
Customers will need to log in with Singpass to verify their identity and complete the survey
While you can send the survey to multiple customers, we will only consider the 5 references you indicate
You may follow up with your customers directly to ensure survey completion
Once you have received 5 completed surveys, update the "No. of surveys completed" field to '5'
Tips: We recommend starting with this step first while working on other parts of your pre-approval application simultaneously.
