How to generate a link

  1. In the General Assessment section of a vendor self-assessment checklist, fill up your Company UEN, Company Name, Solution Name​

  1. In the Satisfaction Survey worksheet, copy and send the link to all the SMEs who are performing the surveys.

  1. To complete your customer satisfaction surveys:

  • Share the FormSG survey link with your customers

  • Customers will need to log in with Singpass to verify their identity and complete the survey

  • While you can send the survey to multiple customers, we will only consider the 5 references you indicate

  • You may follow up with your customers directly to ensure survey completion

  • Once you have received 5 completed surveys, update the "No. of surveys completed" field to '5'
