e-Forms for Pre-school

Allow creation and update of forms to facilitate preschool enrolment, withdrawal and subsidy application for use by parents, operators and ECDA.

Please review all requirements. Your solution must meet all 🔴 mandatory requirements to qualify.

2.01🔴	Can your solution allow for cloud based, mobile based and/or web-based usage?
2.02🔴	Does your solution allow creation and update of forms and include, but not limited to the following forms : a. Child care enrolment and subsidy application - Submission of basic enrolment details (applicable for all children)applying for Child Care Subsidies - Start-Up Grant (SUG) and/or Child Care Financial Assistance (CCFA) (applicable for Singapore Citizen children only - to update change in applicant (for existing enrolled Singapore Citizen children) b. Subsidy update and Special Approval application (Existing enrolment) - To update child and/or applicant/spouse’s details (for existing enrolled Singapore Citizen children); - To apply / Renewal of Special Approval, Start-Up Grant (SUG) - To apply for financial assistance for child care (CCFA) for children who are already enrolled in the centre (applicable for Singapore Citizen children only) c. Withdrawal from child care centre / Subsidy - Withdrawing child from child care centre; or - Withdrawal from Subsidy scheme
2.03🔴	Does your solution allow creation and update of below form : a. Form for Kindergarten enrolment - Provides basic enrolment submission details (applicable for all children) - Supports Kindergarten Fee Assistance Scheme (KiFAS) Note : KiFAS is only applicable for Anchor Operators (AOPs) and Ministry of Education (MOE) Kindergarten only
2.04	Does your solution allow creation and update of Annexes for collection of additional information that may include, but not limited to the following : a. An annex to collect other optional information such as allergies, grandparents contacts etc.
2.05🔴	e-Signature / Digital Signature - Does your solution support workflow that allows routing of forms for e-Signature / Digital Signature? - Does your solution capture the audit trail information as part of the Forms e-Signature / Digital Signature workflow? Note : Audit Trail should include but not limited to the following data : - Date Timestamp - location - IP address
2.06🔴	Export of Data - Does your solution support form data to be exported into acceptable formats (e.g. CSV, Excel sheet etc) for uploading into other systems for processing, i.e. preschool’s centre management system - Does your solution support form to be generated into e-copy version for auditing purposes?
2.07🔴	Upload of Supporting Documents Does your solution support the uploading of documents (File size up to 8 MB) from parents to facilitate the enrolment of child?
2.08	Does your solution provide APIs for sharing and/or retrieving of information with external platforms/solutions? e.g. able to link to MyInfo to autofill personal detail, able to link to ECDA system (If yes, briefly describe the APIs available in the Comment Section or submit a brief write-up together with your checklist submission)
2.09🔴	If your solution comes with the following - Collect Personal Identifiable Information and/or - Sensitive Personal Information (SPI) Has your company engaged a qualified 3rd party to conduct a security vulnerability assessment of your solution in the last 12 months? If you are a reseller of the solution, please verify with your product principal that they have conducted a security vulnerability assessment of their solution by a qualified 3rd party in the last 12 months. If Yes, please indicate the name of the 3rd party assessor and date of the assessment test in the comment field, and also submit the assessment test report as supporting document. Note: [1] Qualified 3rd party include assessors appointed under IMDA GoSecure Programme; Cybersecurity companies accredited under Accreditation@SGD Programme; and CREST-Certified companies or companies with equivalent certifications
2.1	Is your solution currently integrated with the pre-schools’ existing IT systems ? (If yes, briefly describe the integrations currently available with the pre-schools' IT systems in the Comment Section or submit a brief write-up together with your checklist submission)
2.11	Does your solution support invoice and receipt generation (support for ePayment invoice/receipt)?
2.12🔴	Does your e-Signature / Digital Signature document comply with IMDA’s Electronic Transactions Act (ETA) (Cap 88)?
